An individual care plan is designed just for your loved one.

Initial assessment interview

Our process begins with a free in-home assessment interview.  We'll discuss your loved one's specific needs, as well as share information about our professional caregivers and the services we offer.  Some of the topics we will discuss during this assessment interview are:

  • Overview of our company, including our mission,  services and staff
  • Your loved one's care needs and care plan design, including schedule
  • Goals of the service for the family
  • Implementation process
  • Ongoing follow-up plan
  • Pricing and payment methods

Registered Nurse visit and Care Plan design

After the initial assessment interview, our Director of Client Care (a Registered Nurse) will meet with you to conduct a full nursing assessment and design a personal care plan just for your loved one.  The care plan will then be entered into our computer system.

Caregiver selection

Next, we'll work to select one of our professional caregivers who's a perfect fit for your loved one. We categorize our caregivers' experience, skill set and personality type and use that information to ensure a compatible match.  You will have an opportunity to meet the caregiver we’ve carefully matched with your loved one to ensure a positive and lasting working relationship.

Service implementation

Once we have a good caregiver match, services will commence (usually within a few days of the initial assessment). A representative from CarePoint Home Care will contact you to confirm the caregiver, care plan and service start date.

Ongoing follow-up

We are committed to delivering quality and compassionate care. Our Director of Client Care supervises your caregiver, conducts periodic nursing assessments and monitors your loved one’s care to ensure it stays in line with his or her changing needs. If there is an emergency or scheduling issue, we have someone on-call 24 hours a day, 7 days a week that can handle the situation. We also have a continuous supply of excellent caregivers who are ready in case your caregiver falls ill or has an emergency.  We guarantee that a caregiver will be present for every scheduled shift, every time.
